As per the EE Department Teaching Policy as spelled out at http://ee.stanford.edu/teaching_policy.php,
this form should be filled out when requesting a teaching leave for an academic quarter consistent with the policy.
A list of current qualifying courses may be found at http://ee.stanford.edu/qualifyingclasses.php.
This form can be printed and filled out by hand, or cut and paste the
ascii text into a text processor, complete the form, and email the plain
text to vicechair@eemail.stanford.edu.
