Instructions

The instructions to the assistant who prompts the radiologist and completes the form. These should be expanded shortly to include other chores such as loading and unloading the films and measuring. Also available in postscript form.

Observer Form

The basic form for data gathering, filled in by the assistant. The form has three pages. The first is mostly identification and is all that is filled out for images with no associated findings. Also available in postscript form.

Questions

Questions asked by the assistant of the radiologist while filling out the form. Also available in postscript form.
